Denethor, the Steward of Gondor

I have a quick question. When Denethor is eating meat and fruit and it changes from that scene to the scene of his son charging into that suicidal attempt to re-capture the city, but is that blood running down his chin? Was Peter Jackson trying to show the audience he was really crazy by showing him biting his own lips and inside of his mouth and have the blood run down and he didnt even care or notice it? Thanks
